Sleep Clinic — for referring physicians

Request a sleep study

This page helps general practitioners and other physicians request a polygraphy (PG) or polysomnography (PSG) at the Sleep Clinic quickly. You fill in a structured referral letter, turn it into a PDF in your browser, and book the appointment yourself in Nexuzhealth Consult.

How it works

  1. Choose the study: polygraphy or polysomnography (or let the sleep physician decide).
  2. Fill in the referral letter and download the PDF — everything stays in your browser.
  3. Book the appointment in Nexuzhealth Consult and attach the PDF.

Polygraphy or polysomnography?

Polygraphy (PG)

Home recording of breathing, oxygen saturation, heart rate and body position, without EEG. Suitable when the pre-test probability of uncomplicated obstructive sleep apnoea is high: loud snoring, witnessed apnoeas, daytime sleepiness, high STOP-BANG score.

Polysomnography (PSG)

Full recording in the sleep laboratory, with EEG, sleep stages, arousals and leg movements. Indicated when the pre-test probability is lower, when there is relevant comorbidity (heart failure, stroke, COPD, neuromuscular disease, opioids) or when another sleep disorder is suspected.

The decision aid on the form suggests a study type based on the STOP-BANG score and comorbidity. The sleep physician reviews every request and may change the type of study.
